Confined space fatality investigation

In 2001, we were involved in the investigation into the cause of a double confined spaces fatality at the Kelso Reef Pontoon. Our role included meticulous data collection, analysis, and presentation of findings at the Coronial Enquiry. This complex case demanded a thorough understanding of confined space hazards and regulatory requirements, demonstrating our ability to navigate high-stakes investigations and provide expert testimony.

Noise assessment on coal seam gas drilling rigs

We conducted comprehensive personal and area noise surveys at coal seam gas drilling rigs, culminating in the creation of detailed area noise maps. This project involved deploying advanced monitoring equipment in a challenging industrial environment to accurately assess noise exposure levels for workers and the surrounding areas. Our findings enabled the client to implement effective noise control strategies, ensuring compliance and worker safety.

Spontaneous combustion investigation

We investigated the cause of a spontaneous combustion incident involving rags in a spray painting workshop. This investigation required a deep understanding of chemical reactions and material properties to determine the root cause. Our thorough analysis helped the client prevent future occurrences and improve their safety protocols, highlighting our expertise in industrial hazard identification and mitigation.

Volatile organic compound survey in mine trucks

Utilising a Drager X-PID portable chromatograph, we conducted a real-time volatile organic compound (VOC) survey inside new mine trucks. The objective was to determine the cause of workers reporting feelings of illness. This project showcased our ability to employ advanced instrumentation for real-time monitoring and to quickly identify potential airborne contaminants impacting worker health in confined environments.
